Email delivery receipts confirm that a message reached a mail server. They do not confirm that it reached an inbox, was not filtered to spam, was not blocked by a security gateway, or was read by a person. The gap between server receipt and actual receipt is significant and has caused enough operational and legal problems that most organizations have stopped relying on email delivery receipts as meaningful documentation.
Fax delivery confirmation is different in kind, not just degree. When a fax platform confirms delivery, it confirms that the document was received by the receiving fax infrastructure at the destination number. An Audit Trail That Does Not Depend on the Other Party
One of the structural advantages of fax over most alternatives is that the audit trail it generates is held by the sender, not the recipient. When an organization sends a fax through Passport, the Enterprise Status Manager logs every transmission with a timestamp, sender, recipient, and delivery status. That record exists independently of what the receiving organization does with the fax after it arrives.
With email, the sender’s record of a transmission is an entry in the sent folder. The evidence of receipt depends on what the receiving party’s mail server logged and whether that log is accessible. In a dispute, those are two different records held by two different parties, and discrepancies between them are common.
Fax’s audit trail is a single record generated at the point of transmission that documents the event from the sender’s side. It does not require the cooperation of the receiving party to be useful as evidence. For regulated industries where documentation of communication is a compliance requirement, that independence matters.
Cross-Organizational Compatibility Without Technical Prerequisites
Trust in a communication channel also depends on whether it works with the people you need to reach. Fax works with every organization that has a fax number, and essentially every organization in healthcare, legal, financial services, and insurance does. There is no compatibility check, no prior technical relationship, no onboarding process, and no requirement that the receiving party be on a specific platform.
Alternatives to fax, whether EHR-to-EHR messaging, Direct Trust secure messaging, or FHIR-based data exchange, all require that the receiving organization has implemented compatible infrastructure. That requirement is reasonable when the relationship is established and the technical investment has been made. It is a barrier when the communication is with a new counterparty, a small provider organization, a government agency, or any party that has not prioritized that specific integration.

Security That Is Built Into the Protocol
The encryption that enterprise fax platforms apply to transmissions is applied by the sender and does not depend on the recipient’s security posture. When a document is transmitted through Fax 2.0 over the etherFAX network, TLS encryption protects the transmission regardless of what the receiving organization is running. Content is destroyed after delivery using FIPS 140-2 compliant deletion, meaning PHI does not persist in transmission infrastructure after the document arrives.
Email encryption, by contrast, depends on both parties having implemented compatible standards. The sending organization can apply encryption, but if the receiving organization’s email infrastructure does not handle it correctly, the security of the transmission is compromised in ways the sender cannot control or detect.
That asymmetry, that fax security is sender-controlled while email security is joint-dependent, is part of why regulated industries trust fax for their most sensitive transmissions. The security outcome does not depend on the other party doing the right thing.
