Not all fax platforms are the same, and the gap between a basic cloud fax service and a true enterprise fax platform is significant in terms of compliance capability, operational reliability, integration depth, and total cost of ownership. Beyond the BAA, ask specifically how the platform encrypts fax content in transit. TLS encryption over a purpose-built fax network like the etherFAX network is meaningfully different from transmission over standard VoIP infrastructure. Ask whether fax content is stored after delivery and, if so, where and for how long. Platforms that destroy content after delivery using FIPS 140-2 compliant deletion eliminate a category of PHI storage risk that platforms retaining content do not.
Ask about the platform’s certifications: ISO 27001, HITRUST, SOC 2. These certifications are not guarantees of compliance, but they represent independent validation of security management practices that is relevant to your own risk assessment.
Reliability and Network Infrastructure
Fax reliability depends on the network infrastructure the platform uses for transmission. Platforms built on standard VoIP infrastructure inherit VoIP’s sensitivity to packet loss and jitter, which causes fax failures at rates that are unacceptable in high-volume clinical environments. Platforms built on purpose-built fax networks, or using T.38 fax-over-IP protocol with appropriate redundancy, provide substantially better reliability.

Ask about retry logic: how many times does the platform retry a failed transmission, at what intervals, and what alerts are generated when retries are exhausted. Ask about disaster recovery: what happens to inbound faxes during a platform outage and how does the platform behave during a local network disruption at your facility.
Routing and Workflow Capability
Basic cloud fax services assign a fax number and deliver documents to an email inbox. Enterprise fax platforms support sophisticated routing configurations that direct inbound faxes to the correct workgroup or individual based on configurable rules. That capability is what makes the difference between a fax platform that requires manual document sorting and one that supports clinical and operational workflows at scale.
Evaluate the platform’s routing configuration depth. Can routing rules be based on the receiving number, the sending number, time of day, or document characteristics? Can routing rules be updated by administrators without vendor involvement? Can different workgroups within the same organization have separate queues and access controls?
Ask how the platform handles high inbound volume. A platform that performs well at low volume but creates queue backlogs at peak volume is not adequate for hospital or laboratory environments where fax volume surges during clinical peak hours.
Integration Capabilities
For healthcare organizations, the fax platform needs to integrate with existing clinical systems. Ask specifically about EHR integrations: which EHR platforms does the vendor support, how are those integrations implemented, and what is the vendor’s track record with the specific EHR your organization uses. The same questions apply to LIS integration for laboratory environments.
Ask about email integration with Outlook and Gmail, which allows clinical and administrative staff to send and receive faxes from within the communication tools they already use. Ask about API availability for organizations that need to build custom integrations with proprietary applications.

The administrative capabilities of a fax platform matter as much as its transmission capabilities for organizations managing fax at scale. Ask what tools the platform provides for real-time monitoring of transmission status across the organization. Ask how administrators access audit logs, how far back records are retained, and whether logs are searchable by date, number, user, or transmission status.
For multi-site organizations, ask whether the platform provides centralized administration across all sites or requires separate administration at each location. A platform that requires site-by-site management does not provide the visibility and control that enterprise compliance requirements demand.

Ask about the implementation process: how is the platform deployed, what is the typical timeline, and how does the vendor handle number porting for organizations with established fax numbers. Ask about support coverage: what are the support hours, what is the escalation path for critical failures, and does the vendor provide regional support for international deployments.
